Here is every reason I can find and think of to prove my point, but any true fan can tell just by watching the Trailer. Hideo, you've done it once again!
Notice the voice at the end of the trailer saying "V has come to." As in, the person referred to as "V" has awoken. There is all sorts of stuff that can be inferred from that meaning (the 5th Snake clone or they simply call him Patient V at this facility), but the most important thing to not is that the voice sounds just like Olga in her Cyborg Ninja outfit.
Also, the patient with his face fully bandaged totally sounds like Kiefer Sutherland.
Also, Phantom Pain is a reference to the feeling that you still have a limb that you have lost. However, with all the supposedly dead reminders from MGS3, I would say that the Phantom Pain is the remembrance of the events and trauma from MGS3. I couldn't find a picture of this, but when you see the main character looking at his claw for the first time, you can see white flower petals floating around, referencing the final fight with The Boss in MGS3.
I haven't decided if this is supposed to be Solid Snake or Naked Snake. The evidence points more towards it being some crazy trailer for Ground Zeroes, but I refuse to rule anything out at this point.
